Assessment Options

Dr Schultz is able to facilitate both in-person and online Zoom assessments. 

In-person assessments are conducted at Dr Schultz’s office, Level 3, 162 Collins Street Melbourne VIC 3000. 

Bookings

All assessment bookings are made by contacting Dr Schultz’s Practice Manager, Ms Courtney Murray via email or by telephone (0409 200 287).

Bookings for Independent Medical Examinations must be made by the examinee’s legal representative (Client).

Each Booking made by the Client is an offer to purchase Medicolegal Services from Dr Helen Schultz and her associated entities, under these published Terms and Conditions of Engagement.

Dr Helen Schultz may reject a Booking at her absolute discretion.

Booking Fee (Deposit)

At the time of booking, an invoice will be raised for the booking fee (10% of the total assessment fee) to be paid within 3 business days of the booking.This booking fee is non-refundable without at least 10 days’ notice of cancellation of booking.

The booking fee will be refunded from the final booking. This fee then enables Dr Schultz to set aside time to prepare your client for the assessment andread all the accompanying documentation prior to the examination.

Cancellation / Non-Attendance

The Client may cancel a Booking at any time by providing written notice to Dr Schultz, via email.

No Cancellation Fees will apply to bookings that are cancelled 10 days or more prior to the scheduled assessment date. Dr Schultz is more than happy to accommodate change of date requests, or alternatively process a refund of any Booking Fees paid, in these circumstances.

A Cancellation fee of 20% of the total assessment fee (the Booking Fee plus an additional 10% of the total assessment fee) will be charged for all Bookings cancelled within 10 days of the scheduled assessment date.

Bookings that are cancelled within 24 hours of the scheduled assessment, will incur a Non-Attendance Fee of 40% of the total assessment fee.

Examinee Material

A formal Letter of Instruction and all supporting documentation and examinee material must be provided at least 7 days prior to the scheduled assessment date. Failure to provide this documentation will result in the Cancellation of the assessment, with a Cancellation Fee payable.

Dr Schultz does not accept hard copy examinee materials, with all documentation to be provided electronically.

Payment

A Tax Invoice will be issued to the Client following completion of an assessment.

Dr Schultz requires payment for completed assessments to be made strictly within 7 days of the assessment. We ask that all payments are made promptly.

A Late Payment Fee of $50 will apply to Tax Invoices that remain outstanding after 7 days.

Provision of Reports

Dr Schultz will provide a completed report within 10 business days of the assessment.

Dr Schultz is able to provide a completed report in a shorter turnaround time where a report is required urgently. Compliance with these requests is at Dr Schultz’s full discretion, and a rush fee of $400+ GST will apply.

No report will be released until full payment is received.
